if one gets up to go to the bathroom, the other would try to beat them to it. so really, no one can pee. i think panel 4 is this realization.
Panel 4 is both of thm trying to pee or that is trying to need/find the urge to pee
Or perhaps the realization that they're about to race to decide who can urinate the longest, thus avoiding the tab. I assume this whole thing ends in a dine and dash... in a few hours.
